Explanation modes of administration and platforms:
The respondent will complete the EQ-5D. EQ-5D is primarily a self-complete instrument.
An interviewer will read the EQ-5D questions to the respondent and enter their responses because the respondent is either unable to read or write or is unable to be physically present for the interview. May be used interchangeably with existing Telephone- and Face-to-face versions.
This is a legacy version, and in time Interviewer-administered versions will be replacing face-to-face versions. Face-to-face versions were developed for use when participants are able to be physically present during the interview, but are unable to read or write.

For non-commercial use only EuroQol is offering modules for several popular digital survey platforms
These modular EQ-5D versions contain the correct layout for a platform. A separate license should be obtained from its respective owners of these platform to collect data.
Modular versions for REDcap are only provided to non-commercial users. REDCap is a secure web application for building and managing online surveys and databases. It is specifically geared to support data capture for research studies.
Modular versions for Qualtrics are only provided to non-commercial users.
Please note that: EQ-5D in Qualtrics requires a Qualtrics CoreXM Survey Platform, with “Allow Javascript permission” enabled. Without Javascript the survey will not work. Free Qualtrics accounts do not allow Javascript but Professional license and up will. Also bigger non-profit accounts (e.g. universities and hospitals) often do allow it.
Modular versions for LimeSurvey are only provided to non-commercial users. LimeSurvey is an open-source survey software solution. Available as a professional SaaS solution or as a self-hosted system.
